I was a mechanic back in the day and would put my money, based on not seeing it, on a faulty mass air flow sensor. These aren't cheap but can be found at places like pick n pull. It measures the air intake and meters fuel accordingly. The shifting would be due, possibly, to the vacuum modulator on the transmission not getting proper vacuum pressure due to the MAP sensor being inaccurate from inappropriate reading from the MAF.
Again, this is speculation without seeing the car. If you wanna swing by, I'll read the codes.

Okay here's one more hint: the car struggles less if you turn the AC off. In fact if it's about to die and shaking and you turn the AC off it can recover - sort of

Sounds like you have a ground or unexposed power wire that is shorting out the engine.

okay here it goes:
SPARK PLUGS!
(and the timing of those gunking up sort of coincides with me feeling generous and clicking 'yes' to the fuel addititive at the HEB gas station. I wonder if that additive kicked loose a decade of gunk and deposited it onto my old spark plugs? can that be?)

it happened to my truck some wires in the pcm bundle had rbb through causing the rpms to surge. I would definetely start withcleaning the maf and throttle body and then move on to a possible short in the ecm.
