If you do the insurance make sure they are giving you a phone that is as good as your current one. I had to talk to the manager before they gave me one that compared to my original.
I'm pretty sure they send you refurbished ones of the same model. A few guys at work have had similar things happen and they all got the same model of phones. The only thing I'm upset about is that I haven't synched my phone with outlook since August I think which means I'm going to lose a lot of stuff.(personal and work related)
