The best way to look at it is this way. Every thing in your tank could have ich. The rock, sand, coral plugs, etc. Ich feeds off of the fish. It needs the fish in order to survive. If you remove the fish from the equation the lifecycle of ich will be broken. They go through several phases of their life cycle. The cyst stage is what you see on the fish. That is the little white spots that you see. This is when they are feeding on the fish. They then drop off the fish into the sand and reproduce. They can stay in the sand for upwards of four weeks. There have been reports of even longer.

Medication or is the hypo enough. I would rather not give meds if it is not needed

I'd avoid using meds. Copper or hypo are the treatments for ich. If you have other problems, I'd attack them one at a time. Your going to be doing a lot of water changes.

Lowering the salinity fast is not a problem(for fish), raising it is. With hypo treatment, you have to be ON THE DOT at all times. It can't waiver, there is minimal room for error.
Those fish in a 20g? hell no
I can't keep exact salinity without a little bit of a swing due to evaporation, so I use Chloroquin Phosphate. It has worked every time, has a wide therapeutic range but is untestable. It's a one and done treatment and takes 2 weeks but I go 3 weeks... just cause.
I'm not going to go into detail in this post but metronitozole+focus+garlic in food+uv sterilizer will solve the ich problem far more effectively than hypo. I have lots of experience with this. Pm if u are interested in details
