I noticed this morning some weird markings on my blonde naso's back. In the middle of his body at the top near dorsal fin.

Any thoughts on what it could be?
Should i be worried?

Hard to see... How long have you had it? And are there any other tangs or fish that might not be getting along with it?

Had him for two weeks now. I also have a hippo, yellow, and dejardini. They are in a 180 gal. I have recently added a cleaner wrasse within the past few days but dont think he could leave those kind of marks..

OK, 2 weeks. From a LFS? So, he's still new to the tank. Looks like an injury. Either from rock or another fish. Tangs can be very territorial. And a quick flip of that tail can leave a heck of a gash.
Can you move him to a quarantine tank? Probably not...

Tangs like those blue cleaner wrasses from Africa. Do you have any in the tank?

I'd say keep an eye on it. If it gets worse or doesn't improve, you might consider moving it to quarantine and treatment.

Maybe its marks caused by darting into the rock work to hide. I know my yellow tang gets marks like this on it from time to time. It always seems to dart into the smallest opening.
