You can set up a corantine tank if you do not have one or do a water change which will help and run carbon. Im not a big fan of introdusing chemical meds into the main tank so wouldnt recommend that.

Bob Fenner said to try and use a good quality food. The "Spectrum" line he specifically noted.

Typical hole in the head comes from malnutrition from feeding foods missing what the fish needs to stay healthy. Poor water quality and stress. Seems that the previous emails have covered these with food suppliments. One more could be if you have an open tank with lamps exposed to the water(No glass tops) and or multiple power heads, submersible return pump adding stray voltage to your tank. I did a test one time when I had a canopy over my 200 and with MH / VHO / T5HO lamps all on I had 20 volts in my aquarium. When I added a grounding probe the voltage went to 0. Not to get them confussed you can not get shocked from stray voltage only current. If you have current in your tank don't add a grounding probe replace the bad pump or power head leaking into your tank.
