It started Friday afternoon and looks like this today. The side that looks bad (left) was/has always been the big healthy side until this.
Water params are all good, any ideas?

Hey Duc

I had a hammer like yours and lost all of the heads in a branch overnight (looks like your problem). I finally observed a peppermint shrimp picking out the heads. The shrimp ended up finishing off the hammer frag before I could remove it.

Just thought I would drop you an idea!

Doesn't look good, odds are it won't make it. I would take it out and frag off the branck in the right and dip in SeaChem or similar. Maybe a parasite or an infection. Did it fall or get stung by anything lately?

Don't have a peppermint or any new anything but I was thinking the same thing.

Does the R side need to be fragged since it doesn't actually touch?
Hasn't moved/fallen, in fact nothing in the tank or around it has changed.

Really hard to say? If an infection, it probably won't spread, but a parasite would. I saved one once that looked similar, fragged it and dipped it in SeaChem reef dip about 2-3 times over a week or two. I ended up loosing it a few months later though. Once Hammers, Frogspawn, etc. start going it's tough to get them back to health - at least that has been my experience.
