found it. Two-spot Bristletooth, Ctenochaetus binotatus.

Looks like a cole tang to me. The lateral lines look like the ones a cole tang would have.

My first thought when I saw the picture was a very sick (possibly flukes) Kole tang. The fins don't seem to match those on the two spot.

yes he is sick. i have no clue what is wrong with my fish all of a sudden. his eyes have clouded up and wont eat. any ideas?

Have you tried the Garlic treatment for feeding. It may entise him to feed. Thats all I can think of. I'm not a fish expert by no means.

Forget the garlic...not gonna help this guy IMO. Flukes would be my number #1 guess. Specifically Neobenedia melleni, a capsalid type monogean. Faded color and cloudy eyes are tell tale signs of this nasty critter. Treatment is easy though.
Treatment #1
4-5 minute freshwater dip will knock them right off. If you do it in a clear container you can know for sure if that is the problem cause they swell up to many times their size before they fall off the fish. Almost looks like fish scales falling off of them. You will have to be the judge of whether the fsh is strong enough to handle the FW dip. He looks pretty far gone to me though.
Treatment #2
Fluke Tabs from Aquarium Products. One treatment at recommended dosage will do the trick. Although I usually follow up with a second dosage at 1/2 recommended dose after 48 hours. If this is his problem he should look dramatically better within 24 hours after the 1st dosage.

I wouldn't do a freshwater dip. If he is anything like a kole, he will freak out. I killed my kole when I gave him a freshwater dip to treat ich. He got so stressed, the scales in his belly fell off and he was pink. next morning we was belly up.

A temperature and ph adjusted freshwater dip will not kill any tang (or any other fish that I have experience with) in and of itself. I think the biggest problem with FW dips is that people wait too long to do them. If the fish is anywhere near death or very stressed because of illness then sure the FW dip will/might push them over the edge. I dip everthing for 5 minutes upon arrival so I'm pretty confident in what I'm saying.
Also FW dips are not an effective treatment for "ich" (cryptocaryon or amlyoodinium i.e. velvet) IME. May actually make it worse if you dip them and put them back into an infected tank because it washes away much of their slime coat making them more vulnerable to infection. I think FW dips are most effective for flukes and parasitic copepods/isopods, all of which are suprisingly common on newly captured/imported fish.
But yes, judging from the appearance of this fish I would use the fluke tabs instead of the FW dip. Sorry if I wasn't very clear on that in my previous post.
